We offer two types of cards in response to requests from our survivors and family members. Please tell us which type of card you want mailed to you. You can Email your order to info@abin-pa.org or leave your phone number at 1-800-516-8052 for someone to contact you.
EMERGENCY CONTACT WALLET CARD
To Show When You Need Help or Assistance
You are welcome to order a free Emergency Contact Wallet Card. Email your order to info@abin-pa.org or call us at our toll-free number, 1-800-516-8052, and someone will contact you.
Please read both sides of the card below to make sure you want to use this message.
Your name and phone number go on the front of the card. The name and phone number of your Emergency Contact go on the back of the card. Then, you may want to laminate the card. You could carry this card in your wallet and show it if you need help or have to explain your situation. You can ask someone to call your Emergency Contact person. You should realize that you will be telling others about your name, phone number, and disability when you show this card. Please use this card carefully as we are not responsible for what happens afterwards.
